Client Policy & Disclaimer Last Updated: 04-02-25 Welcome to Devoted Massage. By booking a massage or any service with us, you agree to the following policies and disclaimers. Please read carefully before scheduling an appointment. 1. Licensing & Service Area I am a licensed massage therapist in the state of Utah (License #: 8803725-4701) and operate in compliance with Utah state laws. All services provided are only for clients within my local area. If you are booking from another state (including Florida), you acknowledge that this is for educational, relaxation, or self-care purposes only and does not constitute licensed massage therapy outside of Utah. 2. Booking & Cancellation Policy Appointments must be scheduled in advance through our online booking system. Cancellation Policy: A minimum of [24/48] hours' notice is required for cancellations or rescheduling. 3. Health & Safety Clients must disclose any medical conditions, injuries, or allergies before the session. Massage therapy is not a substitute for medical treatment. If you have a serious medical condition, consult your doctor before booking. I reserve the right to refuse service if a session could be harmful to your health or if inappropriate behavior occurs. 4. Liability Waiver By booking a session, you agree that Devoted Massage and its therapist(s) are not liable for any injuries, damages, or adverse effects resulting from the massage or related services. Clients assume all risks associated with receiving massage therapy. 5. Privacy Policy Client information is kept confidential and will not be shared with third parties without consent (except as required by law). Payment information is processed securely via Venmo and accept Cash or Check (made out to Francisco Oliveri) 6. Out-of-State Clients (Including Florida) If you are booking from Florida or any other state where I am not licensed, you understand that: This session is not a licensed medical or therapeutic massage in your state. The service is provided for relaxation, stress relief, and general wellness only. You waive any claims related to unlicensed practice in your jurisdiction.
